Creating a plain paper patient history form to be scanned and analyzed with Remark Office OMR is a great way to streamline the data collection. You can create your own form in any word processing application or form design program. Once you have your form designed you print it out on regular paper. Then you distribute the patient history forms to your patients and they bubble in any known items such as: current illnesses, past illnesses, current or previous injuries and hereditary information. These completed forms are then scanned into Remark Office OMR using an image scanner or a multi-function device (printer/scanner/fax). The software pulls all of the information from the form and enters it into a database. The data can be exported to a variety of different formats in order to be imported into a patient management system.
