The following sample forms were created in MS Word for use with Remark Office OMR. These forms were created following the Remark Office OMR form design guidelines. Remark Office OMR can collect and analyze data from surveys, tests, assessments, evaluations, ballots and many other plain-paper forms. You can create your own forms with any word processor or design tool, print them on a laser printer, and scan completed forms with any image scanner of multi-function printer. Remark will convert your completed forms into data that can be exported to most any analysis package including Excel, Access, SPSS, CSV and more. You can also use our powerful Remark Quick Stats analysis component (included with the software) to analyze your tests or surveys.
